2 years of orthopedic experience required
American Heart Association BLS certification required for all direct patient care personnel
State nursing license required with verification within 30 days of start
Specialty specific competency testing must be completed within a year

Fit Test required
Trinity 10 Panel Drug Screen required within 30 days of start
Trinity Code of Conduct Form required
Trinity I9 form required
Confidentiality and Information System Security Agreement required
Integrity and Audit Services Form required
Non Employee Upload Form required
Trinity Color Vision required
COVID-19 Vaccine proof or exemption required
MMR vaccination proof required
Hepatitis B vaccination series required
Influenza Vaccine optional
2 Step PPD OR IGRA (Quantiferon/Tspot) required
