PT Home Health jobs in Keene, NH are available for 13-week assignments starting ASAP. You will work Monday through Friday, 8-hour days, covering 40 hours per week. Required skills include home health experience and a current New Hampshire PT license.Your responsibilities include conducting patient assessments, developing and implementing individualized treatment plans, providing hands-on therapy, and documenting in Homecare Homebase EMR. You will visit 6 patients per day, managing a caseload of 30 patients, and coordinate care with other disciplines for comprehensive, patient-centered outcomes1.
