What You ll Do

• Implement individualized treatment plans to improve patients' physical, social, and mental functions
• Conduct patient evaluations to ensure quality standards and identify areas needing attention
• Use appropriate clinical techniques and tools for therapy interventions
• Communicate care and discharge plans with patients and team members
• Complete accurate and timely clinical documentation
• Supervise assigned assistants and ensure compliance with facility protocols
• Collaborate with interdisciplinary teams to optimize patient outcomes

What You Need

• Current Hawaii State Physical Therapist license
• Minimum 1 year of home health therapy experience
• Strong clinical skills and ability to work independently
• Excellent communication and teamwork abilities
