In this role, you'll provide critical social work services to both home health patients and clients under a Montgomery County contract. Apply today and discover how MedStar Health can be your next great career move. </p> <p> </p> <p><strong> Key Responsibilities: </strong></p> <ul><li><p> Provide psychosocial assessments and develop individualized care plans with the patient, family, and healthcare team. </p></li></ul> <ul><li><p> Deliver short-term counseling and facilitate access to community resources as needed. </p></li></ul> <ul><li><p> Support patients with discharge planning, nursing home placements, and resolving social barriers to care. </p></li></ul> <ul><li><p> Maintain timely, accurate documentation in compliance with federal/state regulations and agency policies. </p></li></ul> <ul><li><p> Participate in interdisciplinary meetings, committees, and community outreach on behalf of the department. </p></li></ul> <ul><li><p> Foster strong working relationships with internal teams and external partners. </p></li></ul> <ul><li><p> Stay current through ongoing education and training. </p></li></ul> <p> </p> <p><strong> Qualifications: </strong></p> <ul><li><p> Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE)-accredited program. </p></li></ul> <ul><li><p> Active licensure required upon hire: LGSW, LCSW, LCSW-C, LISW, or LICSW. </p></li></ul> <ul><li><p> Minimum 2 years of experience in a healthcare setting. </p></li></ul> <ul><li><p> Valid driver's license and reliable personal vehicle required. </p></li></ul> <ul><li><p> Current CPR, BLS, and NRP certifications from the American Heart Association or American Red Cross. </p></li></ul> <ul><li><p> Home care experience preferred. </p></li></ul> <p> </p> This position has a hiring range of : USD $61,838.00 - USD $111,259.00 /Yr.<img src="https://www.jobg8.com/Tracking.aspx?8oLC%2fwVESyACoQLvA%2f0y87nhKNxE3TB%2bn" width="0" height="0" />
