Social Services

Parkside Health Care Center is accepting applications for a full time Social Services Coordinator.

Job Duties:

Acts as a liaison between the resident, family and staff of the facility for any concerns, which may arise.
Initiates an admission interview and assessment
Determines the Social Service needs and/or interventions and develops a Plan of Care.
Monitors for needs and arranges for Dental, Hearing, vision, podiatry, psychological screenings and assists physicians in building.
Participates as part of the Interdisciplinary team in maintaining a Plan of Care.
May be required to coordinate Interdisciplinary team and family meetings.
Participates as part of the Interdisciplinary team in providing interventions to resolve behavior or mood problems.
Attends Resident Care Conferences and completes appropriate section in MDS with signature.
Assures that the resident is aware of Residents Rights. Assists them in exercising their rights if they desire.
Facilitates and coordinates Discharge Planning to assure continuity of care. Arranges for necessary home health care and necessary equipment.
Maintains a current list of Community resources.
Assists the residents in their transportation needs for appointments.
Encourages residents in maintaining or attaining the highest practicable level of functioning possible.
Coordinates Hospice Services as needed.
May be required to attend resident council meetings
Qualifications:

Must have excellent communication skills, written and verbal.
