Coverage Needed: Oct :00 am " Oct :00 pm Nov :00 am " Nov :00 pm Dec :00 am " Dec :00 pm Jan :00 am " Jan :00 pm Clinical Information: 1 week per month starting on Monday at 7am until the following Monday at 7am in the hospital or weekend per month starting at Friday at 5pm until Monday at 7am. Hospital only. Round on hospital patients. See new hospital patients. See ED patients. Pulm consults EMR: Epic Patient volume: 15-20 patients/day Support staff: 3 providers/2 APPS Take call after hours. Required: Must have IN license to be considered Must be board certified in both pulm and CC CV must be in Month / Year format Must have license in hand at time of submission, or proof that the license application has been submitted Any gaps greater than 30 days will require an explanation on the CV DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active Current Flu Vaccine Must be fully vaccinated for Covid 19 Exemptions only allowed for religious or medical reasons, once approved by the facility.
