RESPONSIBILITIES:-Primary responsibility is to provide medical management to an established patient panel-Aid in continuity of care-Position is supported by Heritage Center for Behavioral and Mental Health, comprised of our behavioral health team of LCSWs, LMSWs, and psychiatric nurse practitioners -Flexible schedule Monday-Sat with earned admin time that has the potential to be completed remotely-See a diverse patient panel primarily consisting of adults -Part-time and full-time scheduling available. We will strongly consider experienced providers seeking a flexible schedule or balance alongside another job QUALIFICATIONS:-NY state-licensed -BE/BC-Willing to commute to the site clinic location and integrate amongst the patient panel to provide care
