Requirements:
  • Board Eligible (to become Board Certified within 2 years of becoming Board Eligible) or Board Certified in Internal Medicine - REQUIRED
  • MI license (or willing to obtain) - REQUIRED
  • DEA (or willing to obtain) - REQUIRED
  • BLS, ACLS (or willing to obtain) - REQUIRED
  • Clean malpractice - PREFERRED Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
  • If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
  • Does the provider require visa support - J1 or H1B?
