This vital role involves overseeing post-discharge transition and stabilization cases, performing medication reconciliation, and proactively managing emerging risk scenarios to prevent adverse events. Shifts are typically scheduled from 9:00am to 5:00pm, providing crucial support to patients navigating complex health challenges after hospital stays. Responsibilities and Duties Manage post-discharge transition and stabilization cases during the critical 30-day window following hospital discharge. Conduct exhaustive virtual medication reconciliation to resolve polypharmacy issues and prevent adverse drug events. Perform urgent, same-day virtual visits for patients exhibiting early warning signs from home biometrics to avoid emergency department readmissions. Oversee patients in their post-operative care transitions. Manage advanced heart failure patients requiring remote fluid status tracking and rapid diuretic adjustments. Provide care for patients with severe COPD and chronic lung disease, including oxygenation monitoring and rescue-inhaler therapy optimization. Address cases of brittle and uncontrolled diabetes with severe glycemic volatility and transitions to intensive insulin/CGM regimens. Manage severe hypertension and CKD overlap cases, carefully balancing medication titrations with kidney function. Support high-risk medical patients whose comorbid depression and anxiety impact their ability to self-manage physical diseases. Act as the clinical lead, directing Clinical Operations Representatives and Patient Care Coordinators to address severe Social Determinants of Health impacting clinical outcomes, such as resource deficits, food insecurity, and medication affordability.
