Requirements:
- Board Certified in OB/GYN - REQUIRED
- Active CA license (or in process) - REQUIRED
- Clean malpractice and license history - HIGHLY PREFERRED
- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
- Is the provider interested in full-time or part-time employment? (note in "Additional Submission Details" field) - REQUIRED
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
- DEA (must be obtained prior to start) (note in "Additional Submission Details" field) - REQUIRED no exemptions
- Active California medical license
- Board Certified
