Work Dates Needed: Ongoing May 2026Preferred Schedule:7am-7pm telephonic call for assigned facility. Onsite coverage 8am-5p. Ideally, a working schedule of at least 5 days per week. They will consider candidates that are unable to commit to 5 days a week (2x-3x per weekWorksite Setting: Hospital/Inpatient. Nursing homes (Skilled Nursing and Nursing Facility)Scope of Work: 15-20 patients per day. The physicians and advanced practice providers deliver comprehensive medical management for patients residing in post-acute care facilities. Services include initial patient evaluations following admission from the hospital, ongoing medical management of acute and chronic conditions, and routine follow-up visits to monitor clinical status and adjust treatment plans as necessary.Licenses, Certifications, Requirements: ABFM, active NY license, BLS/ACLS-requirement, state-controlled substance, DEA, Covid vaccination card, TB test (within last 12 months), procedure logs (past 24 months showing inpatient encounters), CAQH#
