• Schedule: 18 patients per day (PPD), with a minimum of 18 PPD
• Work Hours: Monday-Friday, 8 am to 5 pm
• Patient Mix: Diverse mix of pediatrics and adults
• Decision Timeframe: A decision is typically made within 7-14 days
• Acceptance of DO: We will also accept Doctors of Osteopathy (DO)
• Experience: Advanced Practice Clinicians (APCs) with 3 years of experience in a similar setting are preferred (see other AR)
• Board Eligibility: Board Eligible (BE) candidates will be considered if truly BE (5 years or less since residency)
• DEA Certification: Active DEA needed before the start date
• Credentialing Time: Credentialing takes 30-90 days

• Required Immunizations: The following immunizations are required for credentialing (before the start date):
  • Flu Vaccine
  • TB Status/X-Ray Results (results must be within the 90 days before the start date)
  • Positive MMR Titer or Complete Immunization Series
  • Positive Hep B Titer or Complete Immunization Series
  • Positive Varicella Zoster Titer or Complete Immunization Series
  • Tdap Immunization (must be within the last 10 years)
  • Covid-19 Vaccine is no longer required

• Notice Period: Vendor agree to a 30-day notice for ending all assignments.
