Job Description

  • Coverage Needed:
    • Dates for 2023:
      • October 23 7am -October 27 7pm
      • November 20 7am " November 22 7pm
      • December 18 7am " December 22 7pm
    • Dates for 2024:
      • Jan. 15 7am- Jan. 19 7pm
  • Clinical Information:
    • 1 week per month starting on Monday at 7am until the following Monday at 7am in the hospital or weekend per month starting at Friday at 5pm until Monday at 7am.
    • Hospital only.
    • Round on hospital patients. See new hospital patients. See ED patients. Pulm consults
    • EMR: Epic
    • Patient volume: 15-20 patients/day
    • Support staff: 3 providers/2 APPS
    • Take call after hours.
  • Required:
    • Must have IN license to be considered
    • Must be board certified in both pulm and CC
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
