Procedures:General Surgery, Breast Surgery, Colo-Rectal, OB/GYN, Ortho, Neuro (Spine), Thoracic, GI.
ORs and Case Volume:4 OR's,Case Volume 5500/OB Service of 500.
Backup: Yes/NP

License Requirement:Must be licensed before presentation pending license/Any
