Job Description Job Description Licensed Practical Nurse (LPN)

Schedule: Weekends, 8 hr shifts

Duties and Responsibilities:

• Provide skilled nursing care to patients in their homes
• Conduct comprehensive assessments of patients' health status
• Develop and implement individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and evaluate patients' progress and adjust care plans accordingly
• Educate patients and their families on managing their conditions and promoting wellness
• Collaborate with other healthcare professionals to coordinate patient care
• Document patient care activities accurately and timely

Experience:

• Preferred minimum of 1 year of experience as an LPN in a home health setting
• Experience with tube feeding and managing enteral nutrition
• Knowledge of sterile processing techniques and infection control protocols
• Experience working with children in a healthcare setting preferred

Job Type: Part-time

Pay: $41.00 - $43.00 per hour

Expected hours: 8 - 16 per week
