This role focuses on chronic disease management, care coordination, and helping older adults maintain their health and independence. Responsibilities: Perform initial, annual, and ongoing patient assessments and develop individualized care plans. Manage acute and chronic medical conditions for a frail and elderly population. Collaborate with physicians, nurses, specialists, and the interdisciplinary care team. Provide patient and family education regarding treatment plans and disease management. Coordinate specialty referrals, hospital transitions, and follow-up care. Maintain accurate and timely clinical documentation. Qualifications: Active California Nurse Practitioner and RN licenses. Graduate of an accredited Nurse Practitioner program. Minimum of 5 years of nursing experience. At least 1 year of experience working with an elderly or frail population. Experience in primary care, geriatrics, managed care, skilled nursing, long-term care, or home health preferred. DEA registration required within 6 months of hire. BLS certification required.
