Job Description
Nurse Practitioner Comprehensive Adult Care We are seeking a compassionate, mission-driven Nurse Practitioner to provide high-quality, value-based care to older adults with complex medical needs. This role emphasizes outcomes over volume and close collaboration with an interdisciplinary care team. Responsibilities
- Perform comprehensive adult and geriatric assessments in clinic, home, senior living, and via telehealth
- Manage transitions of care to reduce avoidable hospital and emergency department visits
- Identify care gaps and collaborate with primary care providers to optimize care plans
- Evaluate and manage non-life-threatening acute conditions
- Support chronic disease management, preventive care, and advance care planning Schedule
- Full-time, MondayFriday (8am5pm) Qualifications
- Current Oregon Nurse Practitioner license
- ANCC or AANP certification
- 35 years of NP experience (Primary Care, Geriatrics, Internal Medicine, Family Medicine, or Palliative Care preferred)
- DEA licensure (or ability to obtain) and CPR/BLS certification
- Spanish bilingual a plus Compensation & Benefits
- Up to $150K salary
- $10,000 sign-on bonus
