LPN - Licenced Practical Nurse Monday - Friday 3pm-11pm

We are seeking a Compassionate, experienced, LPN to join our unique, family-owned Memory Care, Assisted Living team! You will be responsible for the assessment, diagnosis, and treatment of residents. This position reports to the director of Nursing.

Responsibilities:

• Administer nursing care to residents.
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate resident families about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate residents' medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• Previous experience in nursing or other medical fields
• Familiarity with medical software and equipment
• Ability to build rapport with residents and families
• Strong problem solving and critical thinking skills
• Day to day operations, scheduling, managing LPN and CNA on shift.
