DESCRIPTION:

• 4 days per week
• 3 clinic days and 1 nursing home rounds and visits
• Varied shifts available
• Credentialing approximately 30-60 days
• Nursing home schedule incorporated into the clinic schedule
• Will participate in coverage for the Palative care team for inpatient services
• Staff; Office manager, full time RN coordinator, 2 full time RN, full time LPN, office scheduling assistant
• Call; 10 days per month including one weekend is expected
