TidalHealth, located in Salisbury, MD , is seeking a NP/PA to join our expanding team at TidalHealth Geriatrics . Position Summary

The Geriatric Nurse Practitioner (NP) provides comprehensive primary and consultative care to older adults across home, outpatient, and hospital settings. This role combines direct patient care with participation in a rotating on-call schedule to ensure continuity of care for the geriatric population.


Base Salary Range- $125,000- $145,000 Commensurate with experience


Key Responsibilities

  • Conduct home visits to assess, diagnose, and manage medical conditions for homebound or mobility-limited older adults
  • See patients in the outpatient/office setting for scheduled visits, annual wellness exams, and follow-up care
  • Provide hospital consultation services on assigned days, evaluating geriatric patients admitted for acute conditions and coordinating care with the inpatient team
  • Participate in a rotating on-call schedule covering nights and weekends
  • Perform comprehensive geriatric assessments (cognition, function, falls risk, polypharmacy, frailty)
  • Develop and implement individualized care plans in collaboration with physicians, family members, and caregivers
  • Order and interpret diagnostic tests; prescribe and manage medications per state scope of practice
  • Coordinate transitions of care between home, office, and hospital settings
  • Document all encounters accurately and timely in the EHR
  • Communicate with families and caregivers regarding care plans, goals of care, and advance care planning
  • Maintain compliance with regulatory, billing, and documentation standards
  • Participate in Interdisciplinary Team Meetings
  • Participate and document Chronic Care Management efforts

Qualifications

  • Master's or Doctoral degree in Nursing from an accredited NP program
  • Current state licensure as a Nurse Practitioner
  • Board certification (AGNP or FNP with geriatric experience preferred)
  • DEA registration
  • Minimum 1-2 years of clinical experience in geriatrics, primary care, or related field (new grads with strong geriatric clinical rotations may be considered)
  • Valid driver's license and reliable transportation for home visits
  • BLS certification required; ACLS preferred

Skills & Attributes

  • Strong clinical judgment in managing complex, multimorbid older adults
  • Comfort working independently in varied care settings (home, office, hospital)
  • Excellent communication skills with patients, families, and interdisciplinary teams
  • Cultural humility and sensitivity to the needs of aging populations
  • Flexibility and adaptability for rotating on-call coverage
  • Familiarity with geriatric-specific frameworks (5Ms, Age-Friendly Health System, comprehensive geriatric assessment) a plus
  • Interest in serving the Elder community in the Lower Eastern Shore and work in the Community

Work Schedule

Tuesday to Friday 7am to 5pm and evening and weekend coverage
