Family Nurse Practitioner (FNP) - Primary Care / Urgent Care

Location: Crystal Lake, IL 60014
Employment Type: Contract
Schedule: Approximately 30 hours per week

Position Overview

We are seeking an experienced Nurse Practitioner to provide high-quality, evidence-based primary and family care in a patient-focused clinical setting in Crystal Lake, Illinois.

The ideal candidate will have at least one year of recent Nurse Practitioner experience in primary care, family medicine, or urgent care and be comfortable practicing independently while collaborating with a dedicated clinical care team.

This role focuses on patient assessment, diagnosis, treatment, chronic disease management, preventive care, health counseling, and care coordination to improve patient outcomes.

Important: Candidates must have prior primary care or urgent care experience. Candidates who do not meet this requirement will not be considered.

Key Responsibilities

• Provide evidence-based primary and family care, including patient assessments, diagnoses, treatment, and disposition planning.
• Conduct routine examinations and perform appropriate diagnostic testing.
• Evaluate and manage acute and chronic health conditions.
• Educate patients on healthy lifestyle choices, preventive care, and chronic disease management.
• Support health screenings, behavioral health initiatives, and wellness programs.
• Coordinate with other healthcare professionals to develop comprehensive treatment plans.
• Manage patient appointments and complete patient follow-up.
• Verify insurance, collect payments, and assist with clinic administrative duties.
• Support clinic opening and closing procedures, maintain inventory, and ensure a clean clinical environment.
• Maintain accurate electronic medical records and follow established clinical and safety standards.

Required Qualifications

• Master's degree from a Family Nurse Practitioner program.
• Current national board certification as a Family Nurse Practitioner.
• Active Illinois advanced practice nursing license in good standing.
• Minimum of one year of recent Nurse Practitioner experience in primary care, family medicine, or urgent care.
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross (ARC).
• National Provider Identifier (NPI).
• Working knowledge of chronic disease management and quality measurement.
• Strong verbal, written, and electronic communication skills.
• Excellent organizational, multitasking, and problem-solving abilities.
• Ability to practice autonomously and collaborate effectively with a clinical care team.
• Proficiency with electronic medical records and clinical technology.
• Willingness to obtain additional state licenses as needed.

Schedule and Availability

• Approximately 180 hours over six weeks, averaging 30 hours per week.
• Typically two to three weekdays, with weekend coverage as required.
• Generally, three out of every six weekends are required.
• Holiday rotations may be required.
• An on-call rotation may be required.

Actual shifts will be determined by clinic scheduling needs.
