Caregiver Support Program - Registered Nurse The Veterans Health Administration is seeking a Registered Nurse for the Caregiver Support Program at the VA Sheridan Health Care System. The Caregiver Support Program includes the Program of Comprehensive Assistance for Family Caregivers and the Program of General Caregiver Support Services. This RN provides services, education, resources, assessments, care coordination, outreach, and support for caregivers of Veterans with complex needs. Remain responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. Assess, plan, implement, and evaluate care based on age-specific components. Coordinate care focused on patient and caregiver education, self-management, and customer satisfaction throughout the continuum of care. Follow established policies, procedures, and guidelines. Collaborate with interdisciplinary team members to influence care outcomes. Provide clinical evidence-based services, interventions, education, and consultation for caregivers and Veterans. Support community outreach, resource development, continuous quality improvement activities, and evaluation of caregiver issues. Support clinical program operations and daily operations as assigned. Function in support of caregivers, Veterans, and family members throughout the medical center and community settings. Conduct interim and annual home visits to caregivers and Veterans approved in the program. Provide assessments by phone and clinical video telehealth. Provide referrals or consults for VA services, community-based social services, and other non-VA entitlement programs. Develop and maintain linkages with community health care and social service agencies. Apply advanced knowledge of services and benefits available to caregivers from federal, state, and private-sector resources. Collaborate with the interprofessional team to manage resources and implement services that improve Veteran outcomes. Support assessment and monitoring, health restoration, wellness promotion, illness prevention, and coping facilitation. Advocate for holistic care through patient and caregiver education and provider interaction. Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and guidelines. Share knowledge with peers, support safe nursing care, and contribute to professional development of health care professionals. Evaluate outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Document broad and complex outcomes at the program or service level. Basic Qualifications: Must be a U.S. Citizen. Non-citizens may only be appointed when qualified citizens cannot be recruited in accordance with VA policy. Must be proficient in basic written and spoken English. Must be subject to a background/security investigation. Must pass a pre-employment physical examination. Must pass pre-employment drug testing and remain subject to drug testing requirements for this testing designated position. Education and Licensure Requirements: Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency. The nursing program must have been accredited by the Accreditation Commission for Education in Nursing or the Commission on Collegiate Nursing Education at the time the program was completed. Foreign nursing education above the associate degree or entry-level nursing education requires formal degree equivalency validation from an accepted evaluation agency. Must have a current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. State, Territory, Commonwealth, or the District of Columbia. Graduate Nurse Technician candidates may be temporarily appointed if they otherwise meet education requirements and are actively pursuing licensure, subject to VA policy. Nurse Grade Determination: Nurse I Level I: ADN or diploma in nursing with no additional professional nursing experience required. Nurse I Level II: ADN or diploma in nursing plus one year of specialized nursing experience equivalent to Nurse I Level I, or BSN with no additional professional nursing experience required. Nurse I Level III: ADN or diploma in nursing plus two years of professional nursing experience with one year equivalent to Nurse I Level II; or BSN plus one year of professional nursing experience equivalent to Nurse I Level II; or MSN with no additional professional nursing experience; or related master's degree with BSN and no additional professional nursing experience. Nurse II: BSN plus two years of professional nursing experience equivalent to Nurse I Level III; or MSN plus one year of specialized nursing experience equivalent to Nurse I Level III; or related master's degree with BSN plus one year of specialized experience; or doctoral degree in nursing or related field with BSN and no additional professional nursing experience. Nurse III: MSN plus two years of specialized nursing experience including one year equivalent to Nurse II; or related master's degree with BSN plus two years of specialized nursing experience including one year equivalent to Nurse II; or doctoral degree in nursing plus one year of specialized experience equivalent to Nurse II; or related doctoral degree with BSN plus one year of specialized experience equivalent to Nurse II. Grade and salary are determined by education, scope, length, and quality of nursing practice experience. Preferred Experience: Two years of experience as a Registered Nurse. Physical Requirements: Must be able to perform all activities of the position without restriction. Requires considerable standing, walking, stooping, bending, reaching, lifting, and turning. Requires repositioning patients and assisting with transfers to wheelchairs or stretchers. Mechanical devices must be used when available for heavy or difficult-to-lift patients. May involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. Requires visual and auditory acuity sufficient to provide safe and effective nursing care. May require working with arms above shoulder level or with the neck fully flexed or extended. Must be able to physically control or protect self from physically abusive patients. May be exposed to infected patients, contaminated materials, isolation situations, operative or invasive procedures, and combative patients. Must be mature, flexible, sensible, able to work effectively in stressful situations, and able to shift priorities based on patient needs.
